Really great videos sharing carers' experience of hospital discharge - with a real focus on what can be done to improve the experience for carers and the person they care for #CarersWeek Caring Together Charity Carers Voice Norfolk & Waveney @NHSNWCCG

Today our first Integrated Care Board (ICB) meeting will take place at 10.30am. The ICB is a new statutory organisation bringing the NHS and partners together locally to improve population health and care. ICBs replace CCGs - watch our first meeting here: ow.ly/N3kH50JIEbW

A key barrier to helping carers is awareness. Awareness for staff to know who the carers are and they can be easily identified. Awareness also for people who don't recognise that they actually are carers - so health and care staff know.

Closing this part of the meeting Tricia says that her hope for this agenda item is that we hear peoples' stories and then we have a dedicated action log to make improvements using recommendations like some of those we've heard today.

Patricia addresses a question on our live YouTube link asking will carers passports be put on digital platforms to help carers be identified? A: that is what we are working to and lots of work around Shared Care Records and an Electric Patient Record is ongoing.
