1/ Grady Hospital Clinic I could feel your grief the moment I stepped into the room. The resident had already told me of your loss. So fresh. So recent. They said you cried on an off for the whole visit. BP controlled. Labs fine. And a depression screen was negative. Yeah.

I end all of my clinic visits with "What did I not ask that I should have?" and then "OK what did we not talk about that you hoped we would?"

Had the opportunity to meet a parent whose child has concurrent hospice care. Through incredible advocacy + support this child has lived years beyond the expected "5 minutes" she was quoted at birth. I asked mom what she would say to the docs who prognosticated early on. 1/

Making sure people who want to know are told that death is approaching means avoiding euphemisms, & checking what they've understood. 'Do you understand?' doesn't test understanding. Try 'Tell me how things seem to you now' or 'How will you explain things to your family?'

In the great blood vessel there was plasma and blood cells too. And clotting factors, the whole crew. And there were 3 blood cell types, passing through pipes. And 2 endothelial tears, in need of repairs. And a little tissue factor, the initial reactor. 1/
