Today is an exciting day, as we launch our very own Derbyshire Community Health Services NHS FT Self Management Facilitator Service pilot in Derby City. This service has the potential to increase independence for so many people receiving care and releasing vital capacity back into community nursing teams. (1/2)

The service capacity will increase over the next few weeks and will also have input into acute bedded care where the self-management facilitators will support early discharge with targeted interventions and reduce demand on community nursing teams. Excited to see the impact (2/2)

Incredible work from our Rapid Response teams during May to meet UCR demand. Over 82% of patients were seen within 2 hours. Hugely significant for not only patients but also reducing pressure on emergency and urgent care services. Hard work from so many making a difference
