In 1929, a 25-year-old doctor cut into his own arm and inserted a catheter into his heart. No permission, no supervision, just sheer defiance. He risked death to prove a theory no one believed. His insane gamble saved millions of lives. Here's his story:

Forssmann was just 25 years old when he made history. As an intern at a small hospital near Berlin, he had an idea: what if doctors could access the heart without surgery? It sounded impossible and his superiors dismissed him outright.

But he didn’t let that stop him and took matters into his own hands. He needed surgical tools and he also needed a test subject. So he convinced Nurse Gerda Ditzen to help, making her believe she would be the first human experiment.

But he had no intention of putting anyone else in harm’s way. Instead, at the last second, he strapped her to the operating table and then cut into his own arm.

With no supervision, no assistance, and no guarantee that he’d even survive, he injected a local anesthetic into the skin of his left elbow. He then made an incision and threaded a catheter into a vein in his arm.

Important findings, but worth noting that individually, major trials (from AFFIRM to FAIR-HF2) missed their primary endpoints. Pooling the data helps. I think it has to do with the definition and also the fact that we treat the consequence (ID) and not the underlying causes.

This was one of my first ever publications after medical school. I'd never have imagined that 16 years later I'd be senior author on my own equivalent book due out this August. Can't wait for this! Akshay Gaur Mark Mills Abdallah Al-Mohammad
