Agility comes from things like rapid response. The team releases, gets feedback from customers, makes changes based on the feedback. If they have to ask for permission to make the change, you’ve lost your agility. Separating "product" from the team is a huge mistake.

The shift in focus (as espoused by Scrum) to empiricism from traditional predictive/reactive/deterministic approaches has yet to catch on in your typical software/product development shop.

SOFTWARE DEVELOPMENT IS NOT MANUFACTURING! There. That felt good. Lean provides some useful stuff (like Kaizen and eliminating waste). Lean metrics are less useful than people imagine. A lot of Lean has no applicability at all. <sigh/>
