Professor Lucy Easthope Shouldn't personalise this too much. Diane Vaughan's first book, Controlling Unlawful Organizational Behavior, is very good on how social structures buffer CEOs from bad news until everything implodes. Good CEOs find ways to bypass blockages but discouraged in textbooks.

This is horrendous.
The Dr who saw this child made the right diagnosis and sent him to the right place…and the system let them down, with someone able to discharge him home without even reading their notes: so little importance was placed in it.
