I never understood this take. So community oncologists who do the vast bulk of oncology care in the world and who have a good idea what their patients want have no say in randomized trials??? I guess leave it to the KOLs who travel weekly while others cover clinical care.

Myeloma needs more therapy de-escalation trials. Won’t happen as long as pharma is running the show but we need to demand it. Do we really need duplicate single arm studies treating healthy people (smoldering myeloma)?

Why docs run late in clinic!

1. More patients than docs. Clinic overbooked so patient care not delayed weeks/months

2. Docs spend more time with patient than time slot given (can’t tell someone they have cancer and say bye in 10 min)

3. Prior patients showed up late
