"No rehab potential" This phrase is used ever such a lot, frequently due to shortage of funding/rehab beds/staff - so rehabilitation beds or places have to be strictly prioritised. However, this then should be stated as the reason, not that the person has no potential at all

"Acopia" This is quite often written in the notes as both the reason for hospital admission and as a diagnosis! Quite sloppy on the professional's part who wrote it. If a person is deemed not to be 'coping' - further, deeper investigation needs to be done to find out how & why

"Refuses to communicate" Another dreadful term used We absolutely must explore all options possible, to address a person's unique needs/styles in order to communicate Environment/setting Alternative & Augmented Communication #AAC systems/devices Language Stress/Anxiety Time

"Bed blocker" This term is given to inpatients deemed medically fit for discharge from a hospital setting, but unable to be safely discharged yet (no care package/home available, awaiting equipment..) Instills guilt & shame in patients themselves They hear Please don't use it

"Difficult patient/parent" This is completely subjective, judgemental & dangerous Will very likely lead to all staff reading that statement in the notes, dismissing a lot of what the patient/parents tell them Perfect example here, from Barrett x.com/therhythmthief…
