Neonatologists: “I dunno, I think I’ll still transfuse premature babies if their platelets fall below 50…. 25 just seems too risky..” Also Neonatologists: “Let’s give this baby some dexmedetomidine and coil their duct.” #neoEBM #neotwitter

What's the difference between absolute and relative risk? Find out in 8 minutes with a fun cartoon video! ow.ly/IobS30sxGlq

Too much neonatal evidence, too little time? Focus your reading: #neoTwitter says the BeNeDuctus trial is 1 of 2 most impactful of 2022! Original NEJM: nejm.org/doi/full/10.10… Open-access #ebneoreviews: ebneo.org/ebneo-commenta… Voting continues 12/29. #EBNEOImpact22

When I was a young doctor I had an attending tell me: "You never tell a patient 'I don't know.'" I strongly disagree. Telling a patient you don't know shows honesty and humility. Then you can tell them you will do research on the matter, or get a second opinion, etc.

Publication alert: infants with BPD develop rapid tolerance to furosemide diuresis. Consider the possibility of diminishing returns during prolonged courses! Open-access to the details in JournalofPediatrics through 2/10/24: authors.elsevier.com/a/1iIQJ55Cs8O8v #neoebm #neotwitter #foamneo

Brian King Dinushan Kaluarachchi Evidence-Based Neo Exactly. Not treatment was randomised, but “the intention of the clinical team”: PDA potential causal factor versus indicator of immaturity. Clinical equipoise essential! Proven successful given an open label percentage of only 0.7%.

FRCPCH Matthew Kielt, MD The BPD Collaborative Even for off-label drugs we need to focus on the evidence more to guide clinicians through consensus (known risks vs potential benefits). Jonathan L Slaughter, MD, MPH NICUofThings Souvik Mitra 100% - even before transfer for an invasive procedure I find it's typically phrased as "they need it closed" as opposed to "it's something we could consider but aren't certain it will help, and could have complications/side effects".

Has anybody tried incorporating NAVA into a respiratory severity score (RSS) outcome or something similar? Seems some sites don't record MAP from the vent in their electronic medical records when on NAVA and not as intuitive calculating since PIP and rate vary

I am concerned about how advocates for NIRS in neonatal care (believers) are pushing its use, in spite of recent evidence that does not support its use. I hope some authoritative body issues a statement to promote evidence-based NIRS use (non-use) to counter-balance these forces.

Arguably one of the more important commentaries in the past year for #neoEBM and #NeoTwitter. A reminder that it's not just the evidence, it's how we process it. Consider the following extracts, then see the last two paragraphs of article for some practical mitigation recs.
