As I try to refine my clinical dx acumen - I have pivoted away from seeing labs being normal vs abnormal (red or not red) to clustering labs and using the degree of abnormality as a diagnostic clue. Let me expand 🧐

I precepted Kevin Qian, MD, PhD in primary care clinic yesterday and learned about his 🧬 discovery today! He elucidated how humans could make metabolically active brown fat. Mirabegron at (probably prohibitively) high doses burns 200ish calories per day using this science. Incredible!

Pictured: the end of an era! This was Kaustav Shah, MD’s last day at the VA Greater Los Angeles Healthcare System. I had the pleasure of working alongside him delivering renal and primary care. I learned from his impressive research at ACP on triaging patient messages. I know you’re off to do great things!

How often are we prescribing HF meds that patients can't take due to💵? If a GDMT rx falls into the EMR and no pt is around to afford it, does it even improve ❤️mortality? No! So proud of my UCLA Internal Medicine Chief Residents co-Firm E residents Neal Dixit, MD and Katie Truong, MD for studying this!

New Interns, medical students, or any providers! Today is my last day of residency. It's crazy to think how far I have come. For your consideration: 5 Things I Wish I Knew Then, That I Know Now Please share with whoever. Photo: A Clueless MS1

Choice quotes from a moving Grand Rounds: “It’s our responsibility to be trustworthy” “Systems don’t care. Only people care.” “We would’ve found something to connect. We just had to look” Trust is the currency of transformative clinical care, for patients and practitioners.
