Thank all for your comments on this interesting case! Main findings include lobular injury, hepatocytic cholestasis, many macrophages, frequent hepatocyte apoptosis and mitosis, scattered portal/lobular lymphoplasmacytic infiltrates, consistent with acute cholestatic hepatitis.

Most likely cause of her acute liver injury is the Sakara diet. Her jaundice started shortly after she started this diet and LFTs gradually trended down after she discontinued the diet. We are not sure what ingredient(s) in the diet was hepatotoxic.

65f, recurrent DVT on Xarelto, acute liver failure s/p liver transplant, before transplant total bilirubin 16, ALP 230, ALT 110, AST 200, platelet 50. US liver cirrhosis. After transplant Tbili 5, ALP 400, ALT 2600, AST 4000.
