The summer weather has hit us here in the East of England. However, despite the heat, our doctors and paramedics are still donning their protective uniforms and heavy boots, and travelling across the region to provide patients with lifesaving care when every second counts.

#CRYOSTAT2trial #CCR23 cryostat2 primary outcome by timing of cryo non-sig difference in mortality per timing of delivery defining the optimum time point - worse outcomes if given early, and worse later...

Interesting discussion from cryostat2 results presentation. Trial primary result = no difference. Major trauma with bleeding is not one disease so looking for one treatment protocol may be an incorrect approach. We would not research to find a treatment that works for โ€˜cancerโ€™.

CRYOSTAT2 results #ccr23: - Early *empiric* administration of high-dose cryoprecipitate to trauma patients activating the major haemorrhage protocol did not improve 28-day mortality. - possible evidence of harm if given very early or in penetrating trauma patients...

In the meantime some cryostat2 data from our #ccr23 presentation. Wait for the paper ofc, but I'd suggest: - If you're thinking about giving concentrated fibrinogen (in any form) up front to trauma patients, don't. - Prioritise getting fibrinogen (FIBTEM/FF) levels back asap

Karim Brohi cryostat2 ๐Ÿ™๐Ÿ™๐Ÿ™ amazing work. Look forward to reading the paper. Helpful to know that cryostat2 doesnโ€™t disrupt motion that once hypoF IS identified that replacement is still beneficial! But love your point about not doing anything empirically to trauma pts! ๐Ÿคฃ seems to be true!

I did warn you this week would be seismic for trauma research & resuscitation! Papers need to be published & digested. Dust needs to settle. Things have changed. Huge thanks to Rob, Emma & the whole Critical Care Reviews team for an incredible meeting & for selecting CRYOSTAT2 ๐Ÿ™๐Ÿผ.

A massive day for UK trauma with the publication in JAMA of two game-changing trauma trials: cryostat2 and UKREBOATrial. Both turning points in how we understand and treat trauma patients. UK-REBOA: jamanetwork.com/journals/jama/โ€ฆ CRYOSTAT-2: jamanetwork.com/journals/jama/โ€ฆ

With so much horror everywhere in the world, we must keep improving trauma care through research and global education. For us at CommsC4TS it's our path to try to make a difference, to try to make these events a fraction less terrible. Until humans stop hurting other humans.

Congratulations to all involved in this study, itโ€™s a great achievement for all involved, including Nikki Curry Simon Stanworth, Ross, a truly excellent clinical trials team and of course Karim Brohi Iโ€™m proud to have chaired the steering committee for you.

Our UK-REBOA trial is out. A huge endeavour with major implications for UK trauma care-showing that use of #REBOA in the ED for exsanguinating haemorrhage did not reduce, and may increase mortality. Huge thanks to the entire trial team. Jan Jansen jamanetwork.com/journals/jama/โ€ฆ

Wonderful (and very generous) write up/run down of cryostat2 and its implications by the Saint Emlyn's team. stemlynsblog.org/cryostat-2-st-โ€ฆ ๐Ÿ™๐Ÿผ
