RANT: Stop lowering sedation dosing for unstable hemodynamics during mechanical ventilation POSSIBLE OPTIONS: -Start a vasopressor -Change up sedation/analgesia -Ensure no other etiologies to low BP -DO NOT USE SUBTHERAPEUTIC SEDATION #FOAMed #Sedation

I cannot think of a single time where involving a clinical pharmacist in the care of a patient didn’t improve care and teach me something. Like, I really can’t. #randomappreciation

4 years of ED +/- crit care overnights has given me insanely valuable skills I couldn't have gotten any other way, always recommend spending (even just a little) time on the dark side

I get it, you want drugs at the ready. But an unlabeled, undated+timed syringe makes me *cringe* as an EM pharmacist. PLEASE label syringes to prevent errors - takes 2 seconds to tape a vial to a syringe for even the most emergent (and unideal) situations 🤷🏻‍♀️

Spotify wrapped (EM pharmacist version) 🎧Spent 8251 minutes tracking down meds "that never got tubed" 🎧Your top 5 consults: - "Can you start some antibiotics" - "This med won't scan" - "Paxlovid interactions" - "Start a heparin drip" - "Can u verify ___ ordered 2 seconds ago"

Which vasopressor should you use post ROSC for OHCA? #UWashEMS isn't ready to commit, but there were definitely differences between the Epi and Norepi cohorts. doi.org/10.1080/109031…

Hi, medical toxicologist here. Administering large doses of naloxone to all patients with opioid toxicity to "totally reverse" them is not a good take. My thoughts below:

“Hey this order is going to kill your patient but idk I am just the pharmacist” *left on read* “So… are we gonna do anything about this” *left on read* Nurse call: “WHY ISNT THIS ORDER VERIFIED???????”

Thanks to AJHP for allowing me to write about the deeply personal decision to leave full time clinical pharmacy & all of the complex emotions that come with it. academic.oup.com/ajhp/advance-a…
