I learned this trick from Dr. Amrita Sethi's POEM lecture at #DDW2023. Sometimes during mucosotomy closure (or a long esophageal tear from food impaction in this case), a 'tunnel' or 'taco' can form. Putting your 2nd to last clip more proximal at the base can stop an endless tunnel.
