Calcified lesions represent one of the most challenging subsets for PCI. Successful treatment is feasible in selected cases through careful planning, data integration, and expert use of available devices. Below is an algorithmic approach to the use of balloons, atherectomy, and

Even though we conducted it, this is not the study that will settle the question once and for all. However, I find this result increasingly plausible and compelling: if you plan a short DAPT after PCI and wish to de-escalate, stop aspirin, not the P2Y12 inhibitor.

Goose vs. Gander! Nice retro CTO PCI by the talented team of Primero Ng + Lorenzo Azzalini. Re: ante vs. retro, what is good for the goose is usually good for the gander. So what’s truly off-limits for retrograde? 🤔 Retro Stingray comes to mind - are there others?

2/ My favorite goose/gander move is #HDR - of course. Inject-and-go if you will.😀 Softening the retro cap can overcome the challenges of poor support when seeking CTO entry. But truthfully, there is no single technique that works every time. Most complex cases look like this!
