Secondary Mitral Regurgitation (SMR): Atrial SMR: Driven mainly by left atrial enlargement and annular dilation, with preserved LV function and normal leaflet motion. Ventricular SMR: Driven by LV dysfunction and remodeling, leading to leaflet tethering and abnormal motion.
