Over a hundred staff including MH practitioners, psychologists, CAPs, community connectors, pharmacists, health coaches (+more) are forming new integrated Primary & Community Mental Health Teams for people with complex and severe mental health at a neighbourhood/PCN level 2/4

A big focus on 'no wrong front door' - teams working seamlessly to ensure patients receive support from staff who are best able to support them, avoiding people 'bouncing' around systems and falling between the gaps. The'll work closely with urgent MH care & talking therapies 3/4

Another focus is reducing #healthinequalities by actively working with communities to reduce barriers and increase access to communities who need it most Mental Health Primary Care Sheffield OTs. It's ambitious. It's needed. It's finally happening. 4/4
