I’m sorry for that. The CFS began using judgment to integrate items in a Comprehensive Geriatric Assessment. Health deficits accumulate to impair high-order function: in mobility, daily activities, +/- cognition. That’s ≠ impairment from single-system disorders in young people.

Well done. Delirium as a preventable cause of dementia deserves study. Much of it arises unrecognized from unnecessarily hazardous routine care of older patients who live with frailty. Polypharmacy, untreated pain, immobilization, and chronic indifference all take a costly toll.

A key question to ask at the beginning of a hospital admission. It directs in hospital function ( mobility and everyday function/ ADLs) and sets the tone for improved discharge outcomes.

I’ve observed rural generalists to offer excellent acute elder care by knowing something about the patient/their family when well, recognizing without alarm/disdain that people can have more than one acute illness, and having the disposition and skill to treat their many needs.

It can be shocking just how little attention is paid to mobility in hospital. Both at the start: no comment on how the person moves now. Silence on what they were like at baseline. How can we help people “get better” (or home) if we don’t know what better looks like for them?

Decision-making can be enhanced by offering treatment trials, with actionable guidance on better/worse. What’s murky in the ED can become clear within 48-96 hours in the ICU. When facing undoubted uncertainty we should substitute dynamic decision-making for all-or-none decisions.
