TNFi奏効してる人に構造破壊進行抑制でルーティンでNSAIDs入れる必要性は否定されたが、一方で一部の例（この研究でそれがどの群かは示せてないけど、現実的には効いてるけど炎症が残っていそうな例など）ではNSAIDs add onが疼痛だけでなく構造破壊にも意義がある可能性はまだあり、選択肢になりうる

➡️65 yo now! On a TNFi, AS has improved.

素人質問なのですが、SpAってNSAIDs 1stからの治療無効時にTNFiなど足していくものと思っていましたが、最近はTNFi 1stなのでしょうか？

TNFi入れる時も、基本はNSAIDsをベースにした上で上乗せして入れるものだと思い込んでいました。

TT(腎臓内科×膠原病内科) 「TNFiうまく行ってそうな中でNSAIDsも入れとくメリットが構造破壊の面であるの？」という疑問に答えるためのやや特殊な設定ではありますね。
ツイートで最初に書くにはプロトコルも色々ややこしく端折ってしまってましたが😥

Donald Thomas, MD Avin Maroof One of my lecturers lived by this. Tested ANA in all RA patients and wouldnt start a TNFi in them if the ANA was significant.
