THSD7A-distinctive membranous nephropathy (MN) antigen

Positive THSD7A shows lumpy coarse granular glomerular basement membrane (GBM) staining.

Looks like deposits of immune-complex GN

THSD7A is unlike other MN antigens that show perfect granular GBM staining, eg, PLA2R, NELL1

New antigens of membranous nephropathy kind of day.

1. 70-yr old man, S Cr 1.4, proteinuria 9 g.
NELL1-MN

2. 44-yr old woman with Sjogren syndrome, S Cr 0.8, proteinuria 5g.
EXT-MN

3. 69-yr old man, S Cr 1.0, proteinuria 6g.
NELL1-MN

Both NELL1= no tumor or drug history.

An interesting case of an overlap of anti-GBM disease with membranous nephropathy:

Granular IgG staining along the glomerular basement membranes can mask underlying linear IgG staining and IgG subclasses can help. Linear IgG staining is brought out with IgG1 with the MN IgG4.

Two biopsies today- the diagnosis was made before labs were available.
1. AL amyloidosis. PAS stain showing PAS negative acellular amyloid deposits. Clinical impression membranous. Monoclonal Ig not done.
2. Crescentic & necrotizing GN. Huge crescents everywhere. ANCA not done.

Sometimes a simple stain can make a pathologist’s day. In this case, positive NELL1 in a case of membranous nephropathy today.

Dx: NELL1-associated MN.

79 yr-old smoker with 2 grams proteinuria. Cr 1.7

Will need malignancy work up, as NELL1-MN often associated with malignancy.
