This video of kids playing 'Crazy Train' on xylophones is seriously the best thing I have seen in AGES!! It's so good it makes me cry.

Be sure to watch until you get to the kids rocking out with solo parts at the end.

I don’t always agree with Mr Yglesias but I did today

My Dad had me convinced that if I studied 8 hr per day in college I might get Cs. (I was not a good HS student)

I was scared. So I studied super hard. I learned quickly that learning was fun. And it keeps you out of trouble

Excellent essay from Vinay Prasad MD MPH on why whole body MRI for cancer screening is almost certainly not going to save your life and could easily cause you much unneeded pain.

Christina Farr, if you do a follow-up invite Vinay!

“You cannot buy the scientific community” 👇🏻👇🏻 Nope. No way. Not possible. Never happens.

$$ has no influence in cardiology. Surely not in oncology either. And gosh, everyone knew the dangers of all along

JACC published my first academic paper. 1997.

Dr K, I wish you and your team well. I pledge to give the new JACC the ultimate fairness every Friday on

Truth be told. I’m hoping for some of that old-school Lown Institute type of critical appraisal —you remember

Had my first FRAIL-AF type query recently. Great to put evidence into action and advise against switching elderly patient stable on VKA onto a DOAC. Thanks to John Mandrola, MD for highlighting this interesting study.

Podcast this morning hampered by no power (T-storms in KY are serious). But worse than no power: home espresso machines don’t work. Low caffeine TWIC today.

The reason why the 2/3rds who said yes are correct is found in Erik van Zwet study on the overestimation of effect in RCTs evidence.nejm.org/doi/abs/10.105…

Chance of replicating a significant P w an RCT that had a p = 0.04 is about 37%

REDUCE-AMI is a hugely important trial, not only for deprescribing but also teaching that (most) trials should have an expiration date

I really like this column. Not only because I agree with it, but also because the author makes a strong argument

Further thoughts on Patient advocates, payments from pharma, and why MRD negativity is about forcing CMS and insurers to pay and not FDA approval/ access

Too much of medical science is simply an exercise in showing something (usually a profitable something) works.

Trials are not supposed to be for marketing. Trials are experiments for learning truths about nature.

Positive or null, it does not matter.

Tweets announcing that you enrolled a pt into a trial are better than industry ads w cakes.

But perhaps it’s best not to announce your belief that the industry product will be a game-changer.

I’m not a trialist, but I don’t think that’s how science should work
