Amazing turn out with over 50 HCPs all engaging in #frailty and #diabetes. Hospital and community specialists, nurses, doctors therapists together in Durham.

The interactive work map cases getting consultants, GPs, nurses and pharmacists discussing assessment and management of #frailty and #diabetes. Teamwork is the only way to improve quality of life for older adults living with diabetes (well actually not just older adults...)
