FUN FACT Earthworms don’t have red cells, but their blood is red. Why? Because they have Hb (Fe-bound heme groups in Hb gives blood its red color), which floats freely in plasma as a 3.6 MDa mega-complex (erythrocruorin), and carries oxygen efficiently underground.

MedTox Fellows: Don't be discouraged when you don't recognize a toxidrome in a comatose patient. This is a typical urine drug screen at our center. It is the exception to ever find a single substance. Meth, ketamine, fentanyl and PCP.

Tox fellows, in over 45 years of caring for TCA ODs, I can't recall ever seeing mydriasis. Mydriasis should be good evidence against a TCA as primary agent. Why do most TCAs not have mydriasis?

Steve Curry (@stevecurrymd) 's Twitter Profile Photo

This. At the level of the pupil, TCAs produce about as much alpha blockade and muscarinic antagonism. Pupils usually unremarkable.

Now is as good a time as any to remind you that you probably owe your life to an objectively evil man. Except… if that's true, was he actually evil at all? This week, we tell a twisted story from long ago…
