A patient presented to the ortho doc with heel pain/plantar fasciitis. He recommended in the treatment plan for her to reduce weight to below 70 kgs. Which is all fine as a directive but the point to note is her present weight which is 107 kg. If only it were that simple.🤦‍♂️🤦‍♂️🤦‍♂️🤦‍♂️

It's often pain of some sort that gets any diabetic to take their sugars seriously. Till then it's ignorance is bliss. Chest pain gets the highest attention probably followed by neuropathic pain. #ObservationsFromTheOPD

Saw a 13 year old girl with acanthosis nigricans (blackish discoloration at the back of her neck) BMI 26 referred by the pediatrician. Her FBS turned out to be 108 and her HbA1c 5.9% Her fasting insulin levels was 65. Unfortunately this is not uncommon.

When we add insulin for high sugars, what we are doing in principle is addressing the relative insulin deficiency. What is not addressed is the state of insulin resistance, without addressing which the cardiac benefits are out of reach. So it's not always about the sugars.

What would be the situation for India I wonder and doctors in training where being put down and insulted is often the norm than exception. Don't know if this is unique to medicine. The same applies to our schools and kids.

Painful diabetic neuropathy that often manifests as burning pain is sometimes hard to treat. It does not always get better with good sugar control and can affect the patient's sleep and leave the patient upset. Treatments take time to work too. And side affects affects some.

Putting a patient on insulin is always hard. Especially ones who are elderly and live alone. Their logistics of managing their own affairs are already stretched to have to deal with this in addition. Prevention is always better than cure but some get to us late.
