But this should be the norm, not the exception. The Grenfell Inquiry does not seem to have a similar steward. There are real questions about who will hold the Government accountable for any recommendations the COVID inquiry makes.

Similarly, the first report from the Grenfell Inquiry (vol 1, p15) noted how lessons had not been learned from the inquest into the Lakanal House fire, published four years before the disaster.

I was struck by ALASTAIR CAMPBELL's recent outrage over the contents of the Grenfell Report; and his incredulity at how public inquiries have such limited means to make change happen. This is a real problem, it's not new, but there are solutions.
