Top Ten Reasons the Prostate Specific Antigen (PSA) is Not a Perfect Test for Prostate Cancer: 1. Benign PSA Bounces Following Radiation Therapy for Localized Prostate Cancer

Allen Edel (@tallallen) 's Twitter Profile Photo

Sean Collins Prostate SBRT @ USF Health I wish all would stop giving the uPSA test. Since ARTISTIC it is unnecessary and is a huge source of anxiety in treated patients. thelancet.com/journals/lance…

I wish some super smart basic scientist could come up with a test to differentiate benign bounce (bb) vs recurrence. Although if you play the odds… chance of bb is 25% < 3y and chance of failure for int risk pt after #SBRT is 0%

Sean Collins Prostate SBRT @ USF Health Can see the career arc now. PSA shows us to cure 95% in IR dz and 85+ in HR. Yet, we move to PSMA and other "predictors" to early intensify. Some 15 yrs later, we revert back to PSA realizing the other stuff wasn't perfect either and really just added costs and complexity.

Sean Collins Prostate SBRT @ USF Health In my experience, certainly within SBRT proton world - the lack of an early good post treatment PSA kinetics is real - it is a significant item at risk / benefit discussion.

Sean Collins Prostate SBRT @ USF Health The retort is this work - actually Collins as an author so I know he knows the data :) In gen, kinetics are very strong but I do think there is more variance in yr 1-2 with SBRT - especially when you push dose to 40 or more. pubmed.ncbi.nlm.nih.gov/34774650/

Allen Edel Sean Collins Prostate SBRT @ USF Health In a lot of posts(all written to quite minimise the value of SBRT?), I clearly understood that you do not believe in radiation oncology… Waiting for an endorsement for our daily work for our cancer patients as evidenced in several papers and guidelines!!
