This is a symptom of toxic system culture where a flawed co- design methodology was adopted by the wrong people. Wrong people=wrong program=multiple amendments. So fix the system culture.

With only about 10% or less natural leaders, all the values and purpose need to be instilled by design. First step could be to implement team framework/ checklists to build teams. What we have now is a variety of pyramids- collection of individuals led by scapegoats.
