HLA allosensitization remains a barrier for heart transplant for many individuals despite desensitization. Heart after liver transplant with domino of the liver into another recipient may be an effective strategy for selected cases.

Notably, all patients in our series were women, who are less likely to be transplanted partly due to increased risk for allosensitization from pregnancies. Additionally, Black individuals are more likely to have higher cPRAs compared to other racial groups.
