Thought of one - all Trusts musts put all telephone numbers/email addresses for all key teams and key people (consultants, clinical leads, clinical directors, bed managers, safeguarding people etc) on the Trust website in a very easy to find way and keep them up to date.

On the list of 'things that should be illegal in medicine but aren't,' starting any sentence with 'remember that patient you saw last week...' is definitely in the top 10.
