In patients with lupus and severe inflammatory arthritis, I'll code them as both SLE + RA (regardless of RF/anti-CCP status), and often treat with TNFi with great effect. I think earlier findings that TNFi can induce anti-dsDNA antibodies made people reluctant to treat this
