Unpopular opinion but the systems based approach in the ICU never made sense to me. Problems based approach gives a true overview “CVS: Hypotension, needing pressors GI: massive lower GI bleed” doesn’t mean much “Hemorrhagic shock 2/2 massive lower GI bleed does” 🤷🏽‍♂️

Parijat Sen 100% agree. Systems approach make a physician into a simple technician, and I see stuff like this: "Heme: transfuse PRN for anemia". It's perfunctory. Problem-based inherently is more cerebral, trying to synthesize signs/symptoms from different systems into a cohesive syndrome

J Dylan Hall, MD Parijat Sen I am seeing this, too. Problem based = conceptualize based on illness script vs System based = conceptualize based on a checklist of things to do.

Maarten Van Hemelen Parijat Sen ultimately there is always tension between highlighting the major problems while not forgetting the housekeeping/minor. Either systems based or problem-based plus some kind of checklist probably work fine.

Parijat Sen ICU transfers are more work than new admissions because I have to reassemble systems into unifying diagnoses.
