How to avoid it? A central database of scientific papers, with journals as overlays. Flagged as plagiarized once, the flag stays. Also, makes it hard to impossible to submit to multiple journals at a time.

IRB / ethical approval advice needed: do you seek IRB approval when running a replication study, assuming the original paper you are replicating already had an IRB clearance and your replication is very close to the original? Can you just wave it by pointing at the original IRB?
