Today we said goodbye to Natalie Wilkinson and wished her luck in her new job. We’ve loved having her on our team and she’ll be missed by both staff and patients. She’ll be amazing in her new role and her new team are very lucky to have her 💙

Great point from Jemma Price - transition is not simply a transfer. We need: 🔹discussion with all professionals and patient 🔹 a bespoke transition including environment - visits 🔹 understanding additional needs including clinic time - dedicated longer slots #SiLVaH2022

🔹We need to discuss consider adult physiology and build in changes to adult care pre - transition 🔹how to we challenge non-engagement- specific policies which differ from standard practice 🔹transition is a monumental change - how can we fund to support #SiLVaH2022
