It’s another busy evening for the team. We’ve just been called to an end of life patient to give some pain relief #BlackCountryHealth247

Rachel Blundell, Advanced Nurse Practitioner, has been with the Rapid Intervention Team for 4 years. She said I love my job, the best part is making a difference to patients and families keeping them in their own environment rather than the unknown of A&E #BlackCountryHealth247

The patients I’ve spoken to this evening have said the team have put them at ease and managed to keep them out of hospital. The Rapid Intervention Team work with other services to keep their patients safe and comfortable at home - avoiding an A&E admission #BlackCountryHealth247

The Rapid Intervention Team work from 8am until 10pm seven days a week. There are exciting plans to expand this amazing service to 24/7 soon #BlackCountryHealth247

Junior Sisters, Rachel Hardwick and Teresa Johns have worked for the night visiting service for over 10 years. In the last 12 months they have completed over 1,100 night visits between them - preventing countless patients from having to go to hospital #BlackCountryHealth247

It’s 6am and a call comes in from James Beattie House in Tettenhall. It’s for an elderly gentleman who has an issue with his catheter. Rachel and HCA, Yvonne attended to him - and again prevented a hospital trip. #BlackCountryHealth247
