#ICURehab Early mobility for sedated patients even intermittently sedated patients is indeed problematic. Rather than light sedation, mobile patients need to be awake- this is sedation cessation.

And the no sedation strategy genuinely works well ONLY if the patient is also mobile and allowed out of bed. We sedate people so they will tolerate being confined to bed 23-24 hours/day.

When you think propofol is compassionate safe care.., realize it appears that way in your short term moment.., but long term later.. it’s created damage like this. Kali Dayton

This is a tremendous effort by the authors to synthesize such a massive body of evidence. Now, would I change my practice based on this and say “let’s keep all intubated patients immobile for the first 72 hours”? What would the Awake and Walking ICU think of their conclusions?

This story is repeated everyday in ICUs everywhere. Time to change usual care- and sitting on the edge of the bed during a “sedation vacation” would not preserve her strength. Awake & Walking in ICU needs to be new standard care Kali Dayton

Multimodal, patient-centred symptom control: a strategy to replace sedation in the ICU thelancet.com/journals/lanre… Why is it so attractive to sedate our ICU patients? We can learn from palliative medicine: Sedation is not a valid goal. Patient’s Symptom control should be our focus.

It’s no surprise that deep sedation continues to be a standard practice if clinicians have misperceptions about what it actually does and want sedated themselves.
