#PBMs are the masters of their own special form of 3-card Monte. They win on #rebates & on #DIRfees . It's a rigged game that favors their profits. It's patients who suffer paying out-of-pocket costs based on artificially high list prices for drugs.

Formulary construction in the US is broken & both PBMs/healthplans AND manufacturers benefit while patients pay the price. As far as I can tell the US is the only country where middlemen control the formulary & make more money w higher priced drugs. PBMs call it 'savings'.
