It's great to see the awards continue. It was a privilege to lead the first awards ceremonies when I was part of the London team. Also great to see some of the faces that were part of those are still there.

It's great to see the #ProactiveCareFramework finally published, a lot of hard work and excellent leadership from both clinical and policy perspectives over the past 4 years. It will have great benefits to the individuals it is focused on.

Great to see both of these documents published. Increasing referrals from both ambulance and TEC partners (TSA (TEC Services Association) ) into #UCR will help to keep people out of hospital and in their own homes, which is often better for experience and outcomes.

Take a look at this new resource which has been developed to provide direction to providers submitting #UCR data to the community services dataset (#CSDS) to map the source of referral to the relevant reason code. future.nhs.uk/CommunityHealt… [log in required]

*New data* Nov 23 (52k) saw an additional 2000 people seen by #UCR services compared to Oct 23 (50k). That's another 2000 people staying at home where they want to be. A massive thank you to everyone in UCR teams for their continued hard work in keeping people out of hospital!
