But medics need to try on patients' shoes, too. Celia shouldn't have to tell her traumatic back-story to convince her doctors she's thought this through.

I tried repeatedly to 'Invite' my anaesthetist and surgeon to have a conversation with me about dying (and loss of capacity). They were extremely uncomfortable and kept 'reassuring' me that it 'wasn't necessary'.
