🔊SPIN-ECHO: same scenario, more beneath the surface. Looks like blood, but it’s not—pseudo-SAH in neuroblastoma with CNS involvement. Without context, we oversimplify.

Radiology in a nutshell: Cutting-edge imaging meets the brute force of metal Experience often teaches you to expect the unexpected, but never shields you from it.

This week’s SPIN-POV: In a case of a pediatric ADEM, always look at the optic nerves. Optic disc swelling, protrusion, neuritis, and perineuritis with periorbital fat stranding and post-contrast enhancement are highly suggestive of MOGAD and can clinch the diagnosis.

This week’s SPIN-POV: In a case of suspected CNS pediatric vasculitis, obtaining chest and abdominal imaging might often help in the diagnosis. This patient had generalised lymphadenopathy and lung parenchymal changes, suggesting an infectious pathology. Nodal biopsy revealed TB.

🧠 #SPINTweetorialWednesday Fulminant SSPE (fSSPE)—the rare, rapidly downhill variant of post-measles encephalitis where DWI restriction can be your earliest imaging tell. Let’s go! Primary deep dive: Prasad et al., Neurol India 2022 → doi:10.4103/0028-3886.338737.

This week’s SPIN-POV: Subpial haemorrhage is an uncommon but distinct neonatal bleed, appearing as a curvilinear collection along the cortical surface with an intact pial margin. It can mimic subarachnoid haemorrhage but shows restricted diffusion in the subjacent cortex.

It’s #SPINTweetorialWednesday 🌀 This week, we peel back the pia to meet an under-recognized neonatal foe — the Subpial Hemorrhage (AJR 2021; Barreto et al.). A hidden bleed beneath the pia, crushing cortex, mimicking subarachnoid but telling a deeper story… 🧵👇
