Recurrent ascites 9 months post #TIPS placement, US showed no identifiable flow within the TIPS stent. Penumbra Indigo CAT-8 through a 10Fr TIPS sheath, used for successful mechanical thrombectomy. Result: excellent flow through the TIPS stent. #IRad Society of Interventional Radiology JVIR

92F with cirrhosis and recurrent lower GI bleeds. Poor TIPS candidate, but #IRad can still help control the bleed. Transhepatic portal access to the IMV, which was used to access the bilateral rectal varices. Varices successfully treated with gelfoam embolization. #stopthebleed

57 y/o with a h/o pulmonary coccidioidomycosis presented after massive hemoptysis. CT scan revealing for active extravasation in the RUL. Successful Onyx embolization of a right bronchial artery arising from an intercostal-bronchial trunk. #stopthebleed #justglueit

MVC. CT with laceration of a horseshoe kidney and active extravasation. Normal renal arteries on angio, but an accessory renal artery off the right iliac was extravasating. Coil embolization to #stopthebleed. Know your variant anatomy and/or always perform aortography in #trauma

Unsuccessful TIPS at OSH for refractory variceal bleeding due to complete portal vein thrombosis. Transferred to University of Arizona VIR for ICE guided transjugular portal vein recanalization, Angiojet thrombectomy, and TIPS creation. Venogram 1 week later widely patent w/ a 4 mmHg gradient

Trauma embolization for splenic laceration after MVC. Great example of a pancreatica magna anastomosing with the transverse pancreatic artery. If you use gelfoam for embolization, you must be beyond these branches or risk pancreatitis! Know your anatomy.

Superior right kidney 8 cm renal cell carcinoma in a poor surgical candidate. IR consulted for treatment. Pre-ablation bland embolization, followed by 7 probe cryoablation. 6 month follow-up MRI showed no evidence of residual or recurrent tumor.

Malignancy related superior vena cava syndrome. University of Arizona VIR consulted for management. Successful SVC stenting with resolution of symptoms and patency at 6 months.

Portal hypertensive gastropathy, presented with severe anemia (Hgb 3.5). Referred to IR for variceal management after EGD. Staged TIPS followed by CATO of the gastrorenal varix, with stable post procedure labs.
