        59 y/o with a Gunther Tulip IVC filter placed 6 years ago. Prior attempt failed. Modified loop snare to free from the wall for snare removal, #filterOUT and #GTFO.         Recurrent ascites 9 months post #TIPS placement, US showed no identifiable flow within the TIPS stent. Penumbra Indigo CAT-8 through a 10Fr TIPS sheath, used for successful mechanical thrombectomy.         Unsuccessful TIPS at OSH for refractory variceal bleeding due to complete portal vein thrombosis. Transferred to University of Arizona VIR for ICE guided transjugular portal vein recanalization, Angiojet thrombectomy, and TIPS creation.
