Hx of relapsing polychondritis now with joint pains, rash and ulcers. ✅SCr 1.2 one week prior➡️4.16➡️5.0➡️5.6➡️6.4 ✅UA 2+ prot 3+bld, 11-20WBC/hpf,>50 RBC/hpf. ✅Micro by neph- full of dysmorphic RBCs/few RBC casts.

Effervescent tubules: 67 yo F admitted for workup of new liver mass, with subsequent worsening of renal function. Renal biopsy with isometric vacuolization of tubular cytoplasm, likely secondary to repeated IV contrast exposure (confirmed) during workup (IV contrast nephropathy).
