Functional evaluation of atrial substrate gives relevant data in AF recurrence with isolated PVs. Atrial late activation mapping demonstrated fractionated EGM in the roof - corresponding to low voltage zone-. This site predicted critical isthmus of roof dependent AT.

In a patient with confluent inferior PVs, we are lucky to demonstrate AF inside single circular lesion set encircling all PVs with ; whereas sinus rhythm in the rest of the atrium. After spontaneous termination, automaticity was observed in the posterior wall.

Is PVI-only approach enough during re-do procedure in a paroxysmal AF with reconnected veins ? “Atrial late activation” map demonstrating fragmented EGM & slow conduction in the posterior wall (low-voltage zone) predicting roof-dependent macro-reentry.
