I know the RCT evidence, but there is not a cardiologist on earth who is telling their 47yo brother to leave this alone and treat with meds. Can definitely have a reasonable discussion about the best method of revascularization, but that's it. Unless you hate your brother.

Hmmmmm, what could the difference possibly be between a patient under 60 who gets a TAVR vs a SAVR? Couldn't be that they're sicker! Naaaaaaaah. But we adjusted ... blah blah blah. Surgeons thinking like surgeons here.
