We see spontaneous fistulas daily but traumatic ones are distinctly less common. This young man developed orthostatic headaches 2 years after a brachial plexus avulsion. Surgery with ⁦Andre Boyke, MD⁩ was able to address both a lateral leak and a fistula ⁦Spinal CSF Leak Foundation

Spontaneous intracranial hypotension is caused by spinal CSF loss in 99.9% of patients. In this girl the cause was not only at the level of the skull base but CSF was exposed to the milieu extérieur resulting in 2 bouts of meningitis ⁦Spinal CSF Leak Foundation⁩ jamanetwork.com/journals/jaman…

Excited to share our new research paper showing how decub CTM can identify the dural tear in extradural arachnoid cysts to aid in surgical repair. We also highlight several similarities with lateral CSF leaks & hint of a possible spectrum. Niklas Lützen ajnr.org/content/early/…

New in JNIS: a venographic classification of the epidural plexus (EP) to guide CVF embo through EP navigation. Success: >90% (Types I–II) → 50% (Type III). Type III = longer fluoroscopy + more EP perforations → prefer segmental vein access. Clinical outcomes highly favorable.

I’ve been reflecting on advances in spinal CSF leak care over the past five years — and I truly believe it represents one of the most rapid and transformative evolutions in modern medicine.
