🔄Personalised care is at the heart of what we do here in the Frailty Service. We complete a comprehensive geriatric assessment with each patient, identifying ongoing health issues or concerns then make an ongoing plan with referrals to other members of the MDT. 👩‍🦳👨‍🦳🧓
