How about this…. Anytime a subspecialist, hospital doc, or anyone else tells a patient or family: “I don’t do that…it’s up to your pcp”… …they have an obligation to reach out to said pcp and explain the issue.

To the neurologist who told my patient he has a balance disorder, and he needs a handicapped parking permit, and “just have your pcp fill out the papers”… …REALLY?????

A brilliant distillation of the state of the perverted art of primary care health care in America. Imagine….if all of these issues were seen as opportunities for improvement instead of impossible burdens. We could accomplish so much Kudos @GIMaPreceptor

Wonderful piece. There should be no system that needs gaming, no codes that need maximizing, no compromise on care. Perhaps the wellspring of action needs to come not only from docs but also (especially?) from the patients whose care is impacted.

A brilliant thread from a colleague who truly represents what we all should want to be. We need a wellspring of dissatisfaction, of anger, from the patients and patients-in-waiting (I.e. all of us) to fix a societal system that’s built on inequity. This can’t go on.

A wonderful piece that reminds us of the ripples that form in the pond of illness. And reminds us to connect with all affected. TY for sharing your experience.

Wonderful piece! “Honor that trust” should be in our minds every day, be it in the drama-filled ICU or ED, or in the more pedestrian, but more subtly dramatic, outpatient encounter.

Of all the procedures that require pre-authorization, low dose ct for lung cancer screening is among the most absurd. Barrier to appropriate care? You bet!

A fantastic guide to appropriate use of patient portals; what should be expected of and by both patients and clinicians; and still yet more examples of how organizations need to support clinicians while fostering effective, empathetic patient care.

I’m so proud of all of the people who make Narrateur possible. And I’m proud to be part of a medical school that values the role of arts and humanism in medicine.

A wonderful piece with many lessons. May I add the wish that systems grant us the time that both we and our patients crave and deserve…and also that we be judged not by the productivity we generate, but by the humanity and heart we bring to our work.

Wonderful reminder of the impact that our words, and how they’re delivered and received, has on our patients. All the more important at a time when patients may get bad news via an electronic portal, alone with their thoughts and feelings.
