Look out for a NICE Guideline 'Thyroid cancer: Assessment and Management' on 19th Dec. An earlier draft version in June '22 had various recommendations for pathology, cytology & thyroid MDT's so publication of the final version of this guideline document is awaited with interest!

NICE Thyroid Cancer Guidance recommendations for more use of needle core biopsy after Thy1 & Thy3a FNA & use of ROSE after Thy1 if non (c) are the most significant for pathology & cytology BUT NCB for Thy3a nodules in most cases is not likely to be helpful nice.org.uk/guidance/ng230

Thyroid tumor with enlarged nuclei showing chromatin clearance, pseudoinclusions and grooves. Classic BRAF-like nuclear atypia. Only problem - this is RET mutated medullary thyroid carcinoma. Take home message: Be careful with MTCs, they may mimic anything.
