Safe systems make it almost impossible to unintentionally cause harm. Eg, sewage and sanitation: if my neighbor defecates drunk, they aren't going to kill my entire family. US DOTs created a system uniquely vulnerable to perturbations in attention, failing ~6 million times/year.
