In chronic cases oblique incision better as would need to extend incision proximally as lazy S. PIN is opposite to the tip of radial tuberosity. Avoid radial/ distal trajectory.LABCN neurolysis in chronic cases as it is stuck in the scar tissue

Independent lag screw is better- directed to catch the key Antero- medial fragment (Antiglide screw for Coronoid Shear fracture). To avoid screw getting intra articular go distal to proximal. Than close the book.

Replying to Lee Van Rensburg Great cases to understand injury patterns involving Coronoid and the management. Case 1 is an example of Varus posteromedial rotatory Injury mechanism (VPMRI) whereas case 2 is an example of posterolateral rotatory injury mechanism.

Case 1 - involves antero medial facet (AMF) of Coronoid and lateral ligament complex - (VPMRI) - Fix lateral & medial column Case2 - RH+ lateral ligament complex + Anterolateral Coronoid ALC AMF is intact. (PLRI) Restore lateral column, medial intact. Fixing ALC debatable.
