Each month, SECAmb normally receive and process around 500 DNACPR and ReSPECT documents. In the last four weeks we have uploaded 4️⃣,0️⃣2️⃣8️⃣! #AdvanceCarePlanning

A new time EVER record... In the eight years that IBIS has been used within the Trust, this month almost 1️⃣0️⃣% of patients who called 999 had a care plan available to support care! 😃 #AdvanceCarePlanning #PersonCentredCare

Our Falls Risk Assessment Tool (FRAT) triages a patient’s risk of further falls, sending an electronic 🔴 P1 referral 🟠 P2 referral or 🟢 P3 notification to a local Community Falls Service and/or the patient’s GP 🏠📲➡️🧑🏻‍⚕️
